That Continues
For some families, this is the next stage of care that began in childhood, as a loved one moves from Pediatric PDN into adult long-term care. For others, it begins when an adult now requires this level of medically complex nursing at home.
Loving Hands provides experienced nursing care at home for adults whose needs go beyond what traditional home health offers. Care is delivered under physician orders and tailored to each patient through an individualized plan of care.

How Care Works
A Simple, Guided Process
Initial Inquiry & Clinical Review
Care may begin with a family inquiry, physician referral, or case manager referral. Our team reviews the patient's needs and gathers the clinical documentation and orders required.
Plan of Care & Staffing Coordination
Care is coordinated based on physician orders and tailored to the patient's specific needs, schedule, and level of support required in the home.
Ongoing Nursing Care at Home
Nursing services are provided in the home by the appropriate nurse or nursing team, depending on the patient's condition and level of care needed.
Transition Support
Transitioning from Pediatric PDN to Adult Long Term Care
Families transitioning from Pediatric PDN to Adult Long Term Care often need clarity, consistency, and support through that next stage of care. Whether a family is already familiar with our team or exploring this transition more broadly, our goal is to help make the move into adult services feel informed, supported, and well coordinated.
THE PROMISE
Continuity through the next stage of care.
